Fairview Physician Associates awards grants to improve care innovation
EDINA (June 9, 2011)--Fairview Physician Associates (FPA) has awarded 15 grants totaling $111,020 to improve care innovation for patients. Funding is part of an annual grant program offered to member clinics by FPA to improve clinical quality, efficiency, patient and family experience.
"Through our Small Grant program and other tools, we work to help our member providers and clinic administrators achieve the best possible care and customer experience at the lowest total cost of care," says William Nersesian, MD, FPA chief medical officer. Recipients and projects include:
Fairview Bass Lake Clinic (Maple Grove): Improve patient satisfaction and efficiency while decreasing costs thorough the use of electronic forms for patient registration.
Southdale OB/GYN Consultants, P.A. (Burnsville, Edina): Enhance care and support of obese pregnant women through focused care coordination, communication and education.
Fairview Brooklyn Park Clinic: Enhance care for patients with diabetes through an interactive, hands-on group diabetes wellness and lifestyle program combining nutrition and exercise education in addition to involving a primary care provider, diabetes educator, optometrist and podiatrist in diabetes education.
Fairview Brooklyn Park Clinic: Support care and education for non-English speaking patients through videos using trained medical interpreters.
Oakdale Obstetrics & Gynecology (Brooklyn Park, Maple Grove, Plymouth, Robbinsdale): Increase identification of women found to have diabetes after childbirth; provide counseling and referral.
Fairview Pine City, Fairview Rush City and Fairview North Branch clinics: Improve health of patients with chronic illness through training in wellness self-management using proven techniques of mind-body medicine.
Fairview Eagan Clinic: Reduce tobacco use by developing tobacco cessation group visit curriculum, including a pharmacist and health coach.
University of Minnesota Amplatz Journey Clinic (Minneapolis); Pediatric Hematology/Oncology: Improve bereavement care for families that have lost a child to cancer through better family support, communication and understanding of the grief process.
Fairview Lakes Clinic (Wyoming), Fairview Southdale Weight Loss Clinic (Edina): Evaluate whether adding the services of a clinical exercise physiologist Improves weight loss outcomes and leads to increased exercise levels and improved fitness as measured by body fat analysis, muscle strength and pedometers.
Obstetric and Gynecology Specialists (Burnsville, Edina): Support and educate women in their choice of breastfeeding their newborns and help to increase the length of commitment to breastfeeding.
Fairview Maple Grove Medical Center - Family Clinic: Increase clinic access for patients with chronic disease through use of virtual visits.
Fairview Riverside Women's Clinic (Minneapolis): Enhance social support, time with provider and cost-effectiveness for prenatal care through use of group appointments for nurse midwifery patients.
Southdale Pediatric Associates, Ltd. (Edina), Implement a systemic approach to improve the management of teens with depression in the primary care setting.
Fairview Rosemount Clinic: Improve patient care, expand clinic access and reduce cost through development of a nurse-run lipid clinic within primary care.
Zero Blood, Inc. Fairview Southdale Hospital (Edina): Use a new approach to identify anemic patients prior to pre-operative exam to allow for treatments before surgery to reduce risk of need for additional procedures.
