CAN DO Healthy Lifestyle Program
Fairview Physician Associates (FPA) CAN DO Healthy Lifestyle Program goes beyond diet and exercise. We focus on the "what and how" of lifestyle change. Our health coaches work with your patients for six months to help them reclaim their health. Patients will learn to make lasting changes in their activity, appetite and attitude, leading to better heatlh.
Our health coaches start with a free overview meeting. Then we set up a one-to-one session with patients to help them create their food and activity plan. Our coaches follow up with patients, usually biweekly, in person, by e-mail or phone.

FPA CAN DO costs only $315 for the six-month program. Fairview employees and family covered by PreferredOne insurance receive full reimbursement after completing 80 percent of the program.
