FAQ’s
How do I or my family member qualify?
Ø Participants can be a FV Employee or dependent insured by Preferred One
Ø Participants can be insured by Medica
Ø Participants can self-pay
Ø Participants may need provider recommendation to participate if you have some pre-existing health conditions
Ø BMI of 30 or greater (exceptions are possible as determined by the health educator)
How do I pay?
Ø If you are a FV employee or dependant ,we will bill insurance a one time fee of $170, all co-pays/co-insurance/deductibles apply
Ø Medica insured persons may participate for free if they qualify for grant (limited availability)
Ø Self- pay $170.00 for the 6 month program
Ø Payment will be collected at the 1:1 session (cash, check)
How does it work?
Ø 6 month program focusing on Healthy Eating and Activity (goal is to lose weight through a healthier lifestyle by changing activity, appetite and attitude)
Ø 2 face to face sessions – one general session (4-6 people) and one 1:1 (1 ½ hours each)
Ø 1:1 focuses on “What is the problem as you perceive it?” “What outcomes do you want?” “What resources do you have or need to get?” “What barriers or obstacles have you experienced or anticipate?” Can Do gives strategies for overcoming these.
Ø After 2 face to face sessions, client checks in every 2 weeks by phone or email
Ø Further face to faces are determined on a need basis determined by client or HE
Ø Self report weight at 1 month and 6 months and other intervals as designated

How do I sign up?
Ø Complete the registration form located on FPA website (www.fpanetwork.org click on Can Do)
Ø Once we receive your registration form, we will call you to schedule you for the general session
