Advance care planning

Have you thought about who would make health care decisions for you if you couldn't make them for yourself?

Advance care planning is the process of discussing your choices for health care at the end of your life with your loved ones and health care providers. Honoring Choices Minnesota hopes to involve all Minnesotans in these important conversations. Learn more about advance care planning by visiting the Honoring Choices Minnesota website. You can document your health care decisions in a legal form called a health care directive. Download and print a free copy of the Honoring Choices Minnesota Health Care Directive.

To help you think about how different circumstances might affect your health care goals and choices, complete the following worksheet. We encourage you to discuss these questions and answers with your health care agents, providers, and loved ones. You may also wish to attach this document to your health care directive.
