FPA Network Providers 
FPA Network infrastructure supports population health efforts through care coordination, medical home certification, medication therapy management and other clinical integration activities. We have partnered with community practices since 1993, we have added practices, expanded member benefits, enhanced performance expectations, and improved quality metric outcomes.

Member levels

Fairview Physician Associates Network (FPAN)
An integrated multi-specialty network, FPA Network comprises more than 3,700 providers. Members include independent primary and specialty care, Fairview Health Services and University of Minnesota Physicians practitioners. Membership includes 65 primary care and more than 173 specialty care clinics in Minnesota’s Minneapolis and St. Paul metropolitan area. As a physician-led nonprofit health care organization, we work to advance community health. We emphasize, measure and report outcomes for clinical quality, patient experience and total cost of care. We negotiate rates and quality incentives for risk-based health insurance products and reward clinics that achieve performance goals.

Fairview Health Network (FHN)
A subset of clinics within FPA Network, FHN includes more than 62 primary care and 98 specialty care clinics. FHN enables us to further enhance clinical integration and participate in defined networks and populations. We negotiate rates for risk-based health insurance products and distribute shared savings to medical practices that meet quality, patient experience, provider engagement and total-cost-of-care goals for a covered population.
